Hospice Liaison

Addus Family company seeks a Hospice Liaison to grow hospice services within the Healthcare & Medical Services industry, supporting the Sales & Business Development team. In this field-based role, you will build and manage referral relationships with hospitals, physicians, and community partners, educating them on hospice eligibility, benefits, and Addus programs. You will coordinate closely with clinical and intake teams to ensure timely, compassionate transitions to care.
